We are located conveniently at 4793 Manhattan Drive in Rockford, located one block east of the intersection of East State Street/ Business US 20 and Alpine Road (see map below).  If coming from out of town, we recommend taking Bypass US 20 to the Alpine Road exit (from Freeport and the west), Highway IL 251 to East State Street (from Roscoe and the north), or Interstate 90 to the East State Street exit (from the east).  Take East State Street to Dempster Road and turn south.  Follow the fork in the road to the left and take the immediate left after the fork.  Pain Management and Rehabilitation Center is on the south side of the road.
